We are currently seeking Registered Nurses for the position of Case Manager to work Full-Time (32-40 hours/week).
Responsibilities:
Perform initial assessment of patient to determine hospice needs and eligibility Provide professional nursing care including the administration of medications and treatments as prescribed by the physician and plan of care Establish Plan of Care and make necessary revisions and updates Communicate with interdisciplinary team and attend IDT meetings Maintain clinical notes and related documentation Counsel the patient and family in meeting nursing and related needs Supervise LPN's and Home Health Aides Participate in On-call Weekend Rotation Qualified candidates must possess a current Ohio Registered Nurse license and a Valid Driver's License.
Minimum of two years of experience in hospice, home health or acute care setting preferred.
