Essential Duties and Responsibilities include the following.
Other duties may be assigned.
Responsible for identifying the medically related social needs and providing (directly or through referral) services to meet those needs of the residents assigned to the SSD/LSW caseload.
Responsible to oversee the social needs and services provided to all other residents assigned to other social worker.
Responsible for acting as liaison between the resident and their family, the administration, other disciplines, and the community at large.
Should have understanding of the Medicaid process and completing the application and document process.
Should be able to complete 9401s in the Provider Gateway.
Should have an understanding of social and emotional factors related to the residents illness, to his/her response to treatment, and to his/her adjustment to care in the facility.
Should be able to identify and anticipate the social needs of the residents and their families, and to provide any necessary follow-up or referral.
Should possess the knowledge of community resources available to the geriatric individual and his/her family.
Should have a basic understanding of current trends in medical social work and knowledge of state and federal requirements regarding social services in long term care facilities, especially all issue of resident's rights.
Advocate and support resident's rights, assist in obtaining guardians and conservators when indicated, and participate in discussions regarding ethical issues related to resident rights.
Evaluate the resident's needs for services, with concern given to emotional and social factors as they relate to medical condition through the completion of the MDS 3.

Evaluate the resident's probable length of stay, and establish a viable discharge plan pertinent to medical and nursing requirements.
Establish a written plan of care for psychological needs for each resident:
assess the needs, set goals, establish a plan of action, and review/revise as needed, but at least quarterly.
Coordinate the social services plan of care with other disciplines in a manner towards restoring and maintaining the resident's capabilities at the maximum mental and physical level.
Participate in the development of a total plan of care for each resident as part of the interdisciplinary care conference team.
Attend care conferences, assure at least quarterly social assessment of each resident, and evaluate individual care plans as to appropriateness.
Maintain current and pertinent social information, assessment records and progress notes indicating action taken to meet psychosocial needs on forms provided.
Establish discharge plan where appropriate, coordinate the discharge arrangements and initiate any needed community referrals.
Confer with the nursing department regarding individual resident problems, provide recommendations, and suggestions as appropriate follow through with corrective action as necessary.
Provide emotional support and liaison to residents and family members on issue of death and dying, and make the Hospice referrals when necessary/requested.
Provide coordination of Psychological Services to residents through completion of referrals, monitoring program and liaison to resident, family and staff.
Should have some understanding of the functioning of the human body in order to understand limitations posed in certain conditions and to prevent aggravation of a resident's physical condition.
Certificates, Licenses, Registrations If Licensed Social Worker, display license and maintain license through continuation credits.
