We are currently seeking a Full-Time Hospice Social Worker.
This position will report to and be supervised by our Social Services Director.
The Hospice Social Worker is an integral part of the interdisciplinary team with a focus on meeting the emotional and social needs of hospice patients and their families.
Job responsibilities include, but are not limited to:
Perform initial and ongoing psychosocial assessments Educate families on community resources and facilitate referrals Provide education on advance directives and assist with completion Assess for risk factors which may impact the course of care Provide ongoing counseling where indicated Complete paperwork in a timely manner in accordance with Medicare guidelines Facilitate the transfer process of patients requiring facility care Participate in Quality Assurance and Performance Improvement processes Complete regular visits with patient/family as identified on the plan of care Ongoing collaboration with the interdisciplinary team, including the Medical Director, regarding patient's and family's needs and concerns Assist patient/families with applying for benefits or community services Educate families on hospice services and philosophy Address patient and family concerns related to end of life issues
Qualifications:
Bachelor's degree from a School of Social Work accredited by the Council on Social Work Education Must be licensed in the State of Ohio At least one year of social work experience in a health care setting, preferably hospice Excellent interpersonal skills Reliable vehicle for the purpose of home visits We are conveniently located in the heart of Middletown, Ohio just minutes away from I-75.
